Notice of Privacy Practices
This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Second Sunrise Therapy PLLC (“I,” “my practice”) is required by law to maintain the privacy of your protected health information, to give you this notice of my legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How I may use and disclose your information
- Treatment. To provide, coordinate, or manage your care, for example consulting with another provider involved in your care, with your permission where required.
- Payment. To bill and collect payment, for example sending information to your insurance plan, including Medicaid, so that services can be covered, or confirming your benefits.
- Health care operations. For the business activities of my practice, such as quality review, training, licensing, record keeping, and working with professional consultants or business associates who protect your information.
Other uses and disclosures that may happen without your written authorization
The law sometimes requires or permits me to share information without your authorization, including:
- when required by Colorado or federal law, including mandatory reporting of suspected abuse or neglect of a child or at-risk adult;
- to prevent or lessen a serious and imminent threat to the health or safety of a person or the public;
- in response to a valid court order, subpoena, or other lawful legal process, subject to the protections the law provides;
- to a health oversight agency, licensing board, or government agency as permitted by law;
- in a medical emergency, or to a family member or other person involved in your care when you are unable to agree and it is in your best interest.
Uses and disclosures that require your written authorization
Other uses and disclosures will be made only with your written authorization, which you may revoke at any time (except where I have already acted on it). This includes most uses of psychotherapy notes, uses for marketing, and any sale of your information.
Your rights
- See and get a copy of your records, in the form you request where possible, as allowed by law.
- Ask for a correction if you believe information in your record is wrong or incomplete.
- Ask for confidential communications, such as being contacted at a different phone number or by a different method.
- Ask for restrictions on how I use or share your information. I am not required to agree to every request, but I must agree to a request not to share information with your health plan about a service you have paid for out of pocket in full.
- Get an accounting of certain disclosures I have made.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- File a complaint if you believe your privacy rights have been violated. You will not be penalized for filing a complaint.
My responsibilities
I am required to keep your information private and secure, to tell you if a breach affects your information, to follow this notice, and to not use or share your information other than as described here unless you tell me in writing that I may. I may change this notice, and the change will apply to information I already hold. The current notice is always available on this page and in paper form on request.
Complaints
You may file a complaint with me using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint or 1-877-696-6775. I will not retaliate against you for filing a complaint.
Good Faith Estimate notice
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.
Under federal law, health care providers must give patients who don’t have insurance, or who choose not to use their insurance, an estimate of the expected charges for services, including related costs.
- You have the right to ask for a Good Faith Estimate before you schedule a service, or at any time.
- If you schedule a service at least 3 business days in advance, you should receive the estimate in writing within 1 business day. If you schedule at least 10 business days in advance, or ask for an estimate before scheduling, you should receive it within 3 business days.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Save a copy of your estimate.
To ask for a Good Faith Estimate, email or call me using the information below. For more information about your rights and the dispute process, visit cms.gov/nosurprises/consumers or call 1-800-985-3059.
Website privacy
This website is informational. It has no client portal, no account sign-in, no comment sections, and no forms that collect personal or health information.
- If you contact me, I receive whatever you send by email or phone. Regular email is not a secure way to share personal health details, so please keep first messages brief (your name, the best way to reach you, and general scheduling).
- Technical information. Like most websites, the services that host this site may collect basic technical information such as your browser type and pages visited, in order to keep the site running and secure. I do not use this information to identify you, and I do not sell it.
- Links to other sites. If you follow a link (for example, to save my digital contact card), that site’s own privacy practices apply.
Please do not use this website to share confidential health information. This website is not a crisis or emergency service.
If you are in crisis or need help right now: call or text 988 (Suicide & Crisis Lifeline), call Colorado Crisis Services at 1-844-493-8255 (or text TALK to 38255), or call 911.
